••• Many underprivileged people do not have access to healthcare systems. Improvement in underprivileged people’s health status thus provides the best impact indicator for the performance of health care systems. Policymakers therefore strive to develop equitable health financing systems. To be effective for the underprivileged, health care services need to consider the quality of not only services in hospitals, but primary care as well. This includes providing adequate medical personnel. International conferences, such as those in Cairo in 1994 and in Paris in 2005, and the Millennium Development Goals in particular have set new targets and standards in this regard. In addition, they address challenges such as the HIV/AIDS pandemic.
